Why would I need a rib to correct my nose in a revision rhinoplasty?

I have a hanging Columella and slightly deviated septum due to a previous rhinoplasty. A well respected facial plastic surgeon booked me for a rib revision. He never said what the rib would be used for. I researched after my consult then called the office to ask why.. His receptionist says they are slammed and email her. They have not responded in six days. My payment in full is due this week. I am concerned.

Rib graft for rhinoplasty revision?   I have done rhinoplasty and revision rhinoplasty for more than 35 years and rarely need a rib graft. Your nose does not appear to need a rib graft at all. See a very experienced revision rhinoplasty specialist who can show you photos of his results in noses like yours without rib grafts.

Thank you for your question about a Rhinoplasty. I cannot speak for your surgeon.  It is not uncommon for surgeons to use cadaver cartilage to build up where there is a deficit.  A reputable surgeon should answer your concerns fully before proceeding with surgery.
 
 To be sure, see two or more experienced, licensed and board-certified providers in your area for a complete evaluation to make sure you are a good candidate and that it is safe for you to have treatment.

It does not appears that you need a rib from the photo you have submitted. My final analysis would depend on a careful examination but you would more likely benefit from septum and/or ear cartilage grafts.

It is difficult to say what your surgeon wants to use the rib for. In the photos the nose looks like it does not need a lot of augmentation. If you are having second thoughts, then you should talk to another surgeon and find out what you need. Do not proceed into surgery if you are not 100% convinced that you know what you are getting and having good customer service. Best wishes

In our practice for 25 years, nasal cartilage is always the primary grafting source when needed for a revision rhinoplasty. Not all revision  rhinoplasties require cartilage grafting techniques. If the nose has been depleted cartilage from previous surgery, then consider using ear cartilage.  Rib Cartilage should only be used as a last resort.

Based on your photos, you do not need a rib graft.  I do noses with and without rib grafts.  They are needed when the nose and ear are not acceptable as donor sites or you need a lot of grafting.  This does not appear to be the case with you.  Seek out a second opinion with a board-certified expert in revisions.  Best wishes.

Dear Strawberry Heaven, Based on your photographs alone I do not see any need for rib grafting in your revision procedure. You may want to have a second opinion especially since your surgeon is so "slammed" and is unable to answer a simple question for you prior to surgery. I consider this a red flag. Based on these pictures I don't know why rib cartilage is necessary for your revision. In over 30 years I have rarely found it necessary to use ribs, even in revision surgery. I prefer using septal or ear cartilage.
