Answer: Be aware that there are two basic methods of correcting upper eyelid ptosis. One is called a conjunctival muellerectomy otherwise called posterior ptosis surgery. The other method is called anterior levator resection ptosis repair. The choice of which surgery to perform requires a detailed consultation that includes a neosynepherine test. Beyond the basic-did you respond to the test, you need a surgeon that looks a little bit deeper than this. Are you at risk for dry eye after your eyelid surgery, have you had prior refractive surgery? Do you have a Herrings' law ptosis such that ptosis on one side is manifest but actually ptosis is present in both eyelids? If this is not uncovered prior to surgery, the unrepaired upper eyelid will become the heavy upper eyelid. Under this circumstance, both upper eyelids need to be addressed. If you respond to the neosynepherine test, should a conjunctival muellectomy be performed? This is a clinical judgment called. In my opinion, probably not because, you clearly have a levator dehiscence ptosis. The most physiologic repair would be to correct the levator aponeurosis directly which also affords an opportunity to control the upper eyelid crease. Ultimately, you might find a low cost provider but this is your face and your eyelid. Experience counts. Repairing a poorly done ptosis surgery is much more expensive than having the work done properly in the first place. I recommend finding the right surgery and the right surgeon before worring about what it will cost. Generally in Los Angeles this type of repair might run from about $10,000 to well over $30,000 depending on the setting where the work is performed include the consultation, the surgeon's fee, the facility fee, anesthesia, and pathology fees.
