You are an ideal candidate for a breast reduction with lift.  If you want me upper pole fullness, an implant can added also, but I would do that at a second stage and only if necessary.

I would consider doing a bilateral breast reduction.  This will reduce the size of your breasts and make them premier.  Liposuction of axilla will help the shape keeping it round.always look for a Board certified plastic surgeon. Good luck!Andrew Cohen MD

It appears you are a good candidate for a reduction. Achieving "small, perky breasts" may not be possible simply because large breasts that are reduced do not maintain that "perky, implant" look that augmented breasts have. Be sure to look at long term ( more than one year) results when you meet with your surgeon. Best wishes.
