Breast Reduction it’s a reconstructive nature procedure as the objective is to eliminate the sagged tissue caused by the excessive weight. Patient has symptoms include of back pain, shoulder pain, headaches and rashes con the breast folds (intertrigo) due to excess sweating. The reduction technique improves the shape, size, position by the lifting effect which also produces a cosmetic result. Every breast reduction regardless the technique carries a risk of nipple blood supply (necrosis) compromise, especially with additional factors such as: mammary reduction >2.2lb (1kg) ; nipple areola complex lifting >3.9” (>10cm), dense and fibrous mammary tissue, breast lift with implant, secondary reduction, BMI >30, smoking, diabetes, mammary scars, malnourishment in post bariatric patients, predisposition to thrombotic events and the use of certain medications. The blood supply of the nipple is mobilized and disrupted during the reduction and lifting, thus the risk of necrosis. This complication can be partial or complete. Both complications are resolvable. When the loss is partial, proper wound care and the use of certain dressings will recover almost all the areola tissue, the only drawback is that it takes some time (3 months). When all the nipple areola complex is lost, the aim objective is to the tissue heal properly and the scaring and inflammatory process subsides. When the time is right several reconstructive nipple techniques are available with good results, also there’s the easy option in having a 3D tattoo. A breast reduction with a lifting more than 15-20 cm, a free nipple graft is recommended. The free NAC graft has also drawbacks: limited graft intake, areola hipopigmentation, lack of sensitivity and projection (it flattens). The type of technique (pedicle or free graft) used depends on the surgeon´s experience and with he feels more comfortable. Patients have to be aware of the complication and accept the risk implicated in this type of procedures in case they decide to have breast reduction surgery.
You have very large breasts and while a reduction would certainly improve the appearance of your breasts, you may not be able to get down to a B cup or even C cup. The larger the breast size and the greater the degree of reduction, the greater the risk for nipple loss.Hope this helps.

Yes you can go down to a D cup safely, however, going smaller in size towards say a B cup will increase the chance of nipple/areola loss and may not look proportionate to your body.
Often with extremely large breasts you can not reduce them to a very small size and maintain good shape. Likely a D or DD.
A breast reduction is always a compromise between a smaller cup size and a shape that is not too flat. It is impossible to guarantee a specific cup size, but you should surely notice a very significant improvement. If your goal is to go as small as possible, you may want to discuss a free nipple graft with your surgeon, which would allow to achieve a more extreme reduction compared to a normal, pedicled breast reduction.
