Recently got consult for fat grafting for breast augmentation and told that using the outer thigh/saddle bag area was NOT a good option due to being a generally bad place for tissue and potential for loose skin problems in outer thigh area, post lipo. I'm mid 40s and in pretty decent shape, with no skin damage in this area. I really want to get rid of the lifelong fat from this area, and see lots of other doctors use it..I'm confused. Is this area possible to use this area with good results?
Answer: Outer thighs lipo gives great results...in experienced hands Outer thighs lipo is usually a home run procedure in my practice and that of other lipo experts. However, it's a lumpy disaster in inexperienced hands. I think it's pretty clear what's going on in the discussion you had with whomever you consulted with. Answer: Yes Lipo targeted on the thighs is more effective for individuals with enough elasticity in their thighs to contour well after the procedure. This will help eliminate a crepey or loose appearance.

Answer: Fat transfer to breast Thank you for your question. If you are considering using fat transfer to increase the size and shape of your breast, using outer thigh/saddle bag fat may work fine for you. If care is done during the liposuction procedure, and you have good skin integrity prior to the surgery, you should have satisfactory results after the procedure. The only problem you may run into, is that you may not have enough fat in that area to increase the size of the your breasts as much as you would want. Good luck in your journey!

You need to make sure that you go to a plastic surgeon who has a great deal of experience in liposuction as well as fat grafting techniques. Answer: Outer thigh Liposuction for fat transfer Liposuction of the outer thighs works very well to somewhat well depending on each individual patient and the exact area involved. If you split the saddle bag into a posterior and anterior half the front half typically responds extremely well to Liposuction while the back side as the transitions into the back of the thigh under the buttocks in an area called the banana roll typically does not respond well to Liposuction because the contour of that area is in large part Created by skin laxity especially in an upright standing position. You can see this for yourself by looking at a mirror and seeing the change in skin contour from an upright standing position to bending forward fully flexing your hips. Another way to determine the impact of skin as it relates to body contour is to pull up on the skin just above your hips fairly aggressively until the skin is tight all the way to the back of your thigh.If putting the skin under tension significantly changes your contour then this tells you the impact skin has on body contour as opposed to fat distribution. It’s a little difficult to explain this with words but can easily be demonstrated during an in person consultation. My practice has been devoted exclusively to liposuction and fat transfer procedures for over a decade. It took me a few thousand cases to figure out how to do these skin pulling maneuvers and exactly what parts of the body are more influenced by skin contour than underlying fat distribution. In your case I suggest you schedule multiple in person consultations with board-certified plastic surgeons in your area who have a proven track record with liposuction and fat transfer. During in person consultations and experienced board-certified plastic surgeon should have no difficulty making an accurate assessment showing you exactly the type of Results you can have by showing you before and after pictures of patients who previously had the procedure with similar skin and fat compositions to your own. And experienced plastic surgeon should have enough before and after pictures to easily find similar patient candidates to depict actual previous results. In my opinion it doesn’t matter where the fat is harvested from for the purpose of fat transfer. Making an accurate assessment is perhaps equally as important as technical expertise when it comes to body contouring. There should be no guessing game as to what type of results each individual patient will have based on their candidacy. An experienced surgeon should be able to show you 20 sets of before and after pictures of the exact area you’re interested in having treated. A very experienced plastic surgeon should have 50 to 100 sets of before and after pictures. You should not be interested in seeing the single best result of their career but instead want to understand what average results look like and get confirmation of how many of these procedures the surgeon has actually performed. Do that by asking the surgeon to show you all of their before and after pictures of this procedure on this anatomic area. Ask the surgeon to show you less than average or undesirable outcomes and have them talk to you about the difference. Consider asking the surgeon to show you a 10 consecutive results or the last 10 results. An experienced competent surgeon should have enough confidence to be able to talk about undesirable outcomes and what their typical results look like. They should also be comfortable telling you if they don’t have experience with a particular procedure or an atomic area. When in doubt put on the brakes and schedule more in person consultations.
