What can I do for under eye wrinkles/bags/loose skin/malar edema? (Photos)

You’re smiling in all your photos to it’s difficult to tell the state of your undereyes.

You’ve been through a long and drawn-out journey and there are a lot of questions I have which can’t obviously be answered on this platform.

But a couple of things:
- lower eyelid edema can be an indicator of a medical condition and not necessarily something “cosmetic”
- be cautious in signing up for a lower blepharoplasty - you may not need it and it could make things worse.

Thank you for the question. Based on the photos, you need blepharoplasty surgery to remove excess skin and fat from the lower eyelids. This can be combined with fat grafting. An in person detailed exam is necessary to assess your under eyes and make definitive recommendations. However, based on your photos, I would likely recommend the following plan: transconjuctival (within the eyelid incision) to address the fat pads and smooth the lower lid contour, a pinch blepharoplasty to remove extra skin (incision just under the eyelashes, and micro fat grafting to create a harmonious transition from the tear trough to the cheek.

It seems like you have been on a very long and probably expensive journey. Based on your photos it seems like you would benefit from having a very minor lower blepharoplasty. This is a very safe and predictably procedure. Consult with a well qualified cosmetic surgeon.
