Based on your photos, you are a great candidate for lower eyelid surgery plus fat transfer. The fat can be removed through an incision in the conjuntivae and then fat is transferred into the tear trough underneath. That makes a nice flat appearance under the eye. Be sure to see a facial specialist for consultation.

Thank you for your question. Bags under our eyes are caused by protrusion of the fat pads of the lower eyelids. This is in part due to aging, and in part due to genetics. The most common method of addressing this is via a lower lid blepharoplasty. Some surgeons reduce the size of the fat pads, while some move them downward to function like a permanent filler. I personally prefer the latter.

You are an excellent candidate for lower blepharoplasty, which is done under local anesthesia in our practice. It’s also likely that a trans conjunctival approach can used to hide the scar.

Just based on your photos, I think that you are a good candidate for lower eyelid surgery to address the bags under your eyes. This surgery can directly address the fat that is causing the puffiness.

Hello and thank you for your question. In my opinion you would greatly benefit from a Quadrilateral Blepharoplasty (Upper and Lower Eyelids). If you decide to not do the upper eyelids, a lower lid blepharoplasty will help to remove those bags and give you a less "tired" appearance. Most expert experienced artistic plastic surgeons have a variety of procedures, both non-surgical and surgical, at their disposal to produce excellent cosmetic results in the appropriately selected patient: temporary fillers, micro-fat grafts, and blepharoplasties. Each has benefits and drawbacks and depends on desired outcome and individual anatomy.

In a patient with your inherited anatomy, a lower lid blepharoplasty including micro-fat grafts to the tear troughs will produce the best and longest lasting cosmetic result.

Following the advice of anyone who would presume to tell you what to do without seeing photos and without taking a full medical history, examining you, feeling and assessing your tissue tone, discussing your desired outcome and fully informing you about the pros and cons of each option would not be in your best interest. You do not appear to have excess skin in your lower eyelids. Therefore, you do not need a blepharoplasty (under eye surgery). Filler by an expert can totally correct what you have and should last a number of years. I have had filler in this area last over 5 years.
