Based on your photos, you are a great candidate for lower eyelid surgery plus fat transfer. The fat can be removed through an incision in the conjuntivae and then fat is transferred into the tear trough underneath. That makes a nice flat appearance under the eye. Thank you for your question. Bags under our eyes are caused by protrusion of the fat pads of the lower eyelids. This is in part due to aging, and in part due to genetics. The most common method of addressing this is via a lower lid blepharoplasty. Some surgeons reduce the size of the fat pads, while some move them downward to function like a permanent filler. I personally prefer the latter.
