I had a cyst in my left cheek after getting my wisdom teeth removed. The cyst was removed 9 months ago, but I still have these "dog ears" at both ends of the scar (which have slightly improved with time but not much). I massage it everyday. My surgeon warns that if I want to fix it, the scar will become much longer. I am not sure what to do at this point because the 2 bumps bother me very much but I do not want a huge scar on my face and possibly risk having the dog ears again.

Answer: Dog Ears at both ends of my scar on my face after 9 months? Hi and thanks for your question. The options appear to be either excising the dog ears which creates a longer scar or some type of dermabrasion or laser resurfacing which might alter the pigment of the skin which is treated. After 30 years performing aesthetic plastic surgery, I would recommend you see a surgeon who is certified by the American Board of Plastic Surgery. Answer: Shape vs Scar After 9 months most of the settling down will have taken place. So you may get a little more improvement with time but it will be slow and they will not go completely.So the choice becomes accept the contour or do something about the prominences.Your existing scar appears to have matured well so lengthening the scar would probably produce the same character of scarring. You already have a significant scar and the dog-ears are the more noticeable aspect. I would have thought that removing the dog-ears surgically would lengthen the scar y about half an inch and improve the contour significantly. I would probably under mine the end of the new scar a little to encourage it flatten.On balance I would be inclined to offer you surgical excision of the dog ears. Some people would say wait a year before revising a scar to allow the maximum time for it to mature and settle but you are pretty close.Good luck
