Got rhinoplasty down using silicone and cartilage. What looked like a pimple. When to get facial to get extraction on to find out from doctor it might be my cartilage protruding out . Been 5 months post surgery. How did this happen ? What should be done ? Who is fault ? Myself or Doctor ? Too early for revision ?

Answer: Graft extrusion Hi,You need to follow up very closely with your doctor. The graft needs to be removed and the nose should be left alone to heal fully. only after the nose is fully healed can you proceed with a revision. Mehdi Sina, MD

Answer: Is this cartilage protruding from my nose after #rhinoplasty with implants? Sorry to read about this issue you are having. Silicone nasal implants are not commonly used anymore by most plastic surgeons due to this risk of apparent extrusion (exposure/erosion of the graft through the skin of the nose). An exposed graft like this is basically technically infected even if it has not caused pus or redness of the nasal skin. It should be removed promptly. Silicone is more likely than your own cartilage to actually extrude like this, though it is also possible with your own tissue.Ascribing "fault" is not really productive nor a goal of doctors answering questions here on RealSelf, for the most part. Generally we are here to educate. Your surgeon may have a lot of experience using grafts and get good results in the typical case; it is not appropriate for other surgeons to state that this is anyone's "fault." Rather, this is a possible known risk of implants/foreign bodies and the graft may have migrated, your nasal skin may be thin, you may have had minor trauma that ordinarily would not be remarkable but might have created a dislodgement, who knows. For you to progress from here, you need the extruding object removed, and you need to then give time for healing and skin softening/remodeling before considering other methods of adding volume to the nose (like diced cartilage/fascia grafts, fat or fillers).
