Hi doctors, I had butt implants for 4 years. Unfortunately the implants were not placed intramascular but under the skin. I removed the implants today and the skin is saggy. I litterly have two big deep dents on both cheeks. The surgeon advices against fattransfer because of the scar tissue.will the garmant help reduce the dents? If I gain more weight is there a possibility that the deformity will be less? What can I do? Thank you
Answer: Butt lift followed by sub fascial implants Dear Stacey, Thanks for posting your pictures and sorry for your problems. From observing your pictures, your butt skin is very loose and the only way to give you a nice youthful butt is to have a butt lift (butt plasty or skin tightening). This entails cutting off the excess skin and the scars will be place naturally along the posterior Iliac bone crest. Based on your pictures the scars should heal nicely, however scar camouflage tattooing is an option as it was elected by few of my patients. Once the wounds heal well , you can have the butt augmented again, however make sure that the implants are place sub fascia and not under the skin. I do not recommend intra muscular placement of the implants because only small to medium size implants can be use and the butt shape is not natural - looks like a bubble. Fat injection to the butt is not a good option, because the fat disappears completely or almost completely within few months or even sooner. I have seen it on many patients (over 150) who came to my surgery center for butt implants surgery after their big disappointment from the fat injection procedure that was done by other surgeons (I do not do fat injection to the butt). Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Check the photo gallery, to make sure that the scars are placed correctly for the butt plasty. For the butt implants, make sure that patients have nice 'S' curve, nice projection, round hips and natural look. Best of luck, Dr Widder

Answer: Dents after removal of buttock implants Unfortunately, your buttock implants were placed on top of the muscle hence depressions, sagging, and “dents“ are much more common when they are removed compared to implants that were properly placed within the muscle. You have to wait at least 6 - 12 months to achieve the final outcome. There’s likely to some improvement although it may be minimal. Honestly, if your only problem was unnatural appearance due to them being on top of the muscle, the best option may be to put implants back in the appropriate intramuscular plane. Glad to help.

Answer: Buttock implants Be sure to wear your compression garment at all times and as much as you can. That will help the skin retract. There are things that can be done to help with the appearance. Be sure to choose a Board Certified Plastic Surgeon that specializes in this procedure and is well known for it.
