I have this long vertical dent in my forehead that completely throws of the symmetry of my forehead (and face in general). I have had Botox, and filler added to this area with very temporary results. I am looking for a long lasting or permanent result now. I recognize that this area of the face is a bit dangerous to work on. What are my best options?
Answer: Forehead indentation Thank you for your question. If you did not experience trauma to that area in the past, then you should perhaps have your medical doctor rule out the forehead indentation as a manifestation of an autoimmune disease (e.g coup de sabre/ scleroderma). Otherwise, the forehead indentation can best be treated with fat grafting or sculptra. In my experience, these two modalities produce the most consistent result for volume augmentation of the forehead and temple areas and it can be longer lasting than injecting fillers. Good luck!

Answer: Vertical crease in forehead Thanks for sharing a photo. That's a very interesting finding, the vertical crease that runs the entire height of your forehead. I don't believe I've ever seen that before. The bone and muscles of the forehead are typically pretty smooth, and when contracted, the muscles cause horizontal lines, not vertical ones. A detailed exam by a plastic surgeon would the first step in evaluating this. I agree, I don't think Botox and fillers are the long-term answer. But you also look young, so I don't know how aggressive you want to pursue a surgical option. I wish you the best! See a board certified PS.

Answer: Treatment options It sounds like your initial approach was a good one.I would differentiate to determine which was more effective Botox or filler's.One option is using longer-lasting fillers.Radiesse it's a much better option than any of the hylouronic acid-based fillers such as Juvaderm.there are even longer lasting fillers such as artefill.fat transfer has the potential of giving permanent results.It is however fickle and can be unpredictable.When working with fat transfer to correct precise volume deficits in the face such as what you have it's better to be conservative and plan on doing a few treatments six months apart.That will minimize the chance of having problems such as a lumpy four head and give you when your surgeon time to fine-tune your result for a lifetime of good aesthetic outcomes.if Botox makes a substantial improvement then continuing Botox is the way to go.There are surgical procedures that can mimic long term Botox effects by surgically weakening the muscles that are injected with Botox.you should interview multiple providers.Specifically look for providers who can prove they have ample experience with full facial fat transfer including the forehead.Most facial fat transfer is done in the mid cheek area giving diffuse volume.This is quite different then the very precise type treatment you're looking for.Best of luck,Mats Hagstrom M.D.
