In the early 1990's Louis Binelli promoted his version of an old operation that was plagued by recurrence and made it more reliable. That involved a permanent suture that helped keep the areola small after removing skin (Before Binelli, doctors used dissolvable sutures that  resulted in near universal recurrence, scar widening and very poor outcomes, thus it was almost never done). It appears that keeping the areola small with a suture for at least 6 -12 months is required and no absorbable suture offers that longevity in strength. Even when using permanent sutures, the areola will spread over a period of 5 years or so in many patients (After 23 years in practice, I can say that). Donut Mastopexy is a technique that is very limited in its ability to address the issues associated with Breast Sagginess and should really be limited to patients with very little breast tissue, who are getting an augmentation and only need the nipple areolar complex moved 2 cm or less to achieve the desired results. If you are that patient, the result can be very acceptable, avoiding the traditional scars of a breast lift. Don't worry about the permanent suture. People live perfectly normal lives with permanent sutures in their body (I have them and am thankful). If they become an issue after 12 months, they can easily be removed under local anesthesia.
