i have this raised fat pad under my eye , i have had it or many year so far not sure if it is due o aging or a bad filler ... shop i see a dermatologist or a facial plastic surgeon to flattened it out ?
Answer: Facial Fat transfer + Lower blepharoplasty From the photos that you have provided, a combination of procedures could be used to help lessen the appearance of your under-eye bags. Although the appearance of the under-eye bag is prominent, your under eyes are also experiencing a bit of hollowness which contributes to the accentuated undereye fat pad. You appear to be a good candidate for a facial fat transfer to help smoothen the transition between your under eyes and lid cheek junction area. Additionally, a lower blepharoplasty could also be considered to help reduce the fat pad under the eyes. Going forward, I would recommend you to complete multiple in office consultations with board certified facial plastic surgeons to discuss your aesthetic goals. This way, they can better assess your face and create an ideal treatment plan for you. Best of luck! Kian Karimi MD, FACS

Answer: Removal of lump after filler? Only on the basis of in person evaluation and your personal history (fillers, kind, time) one could decide what is the most appropriate treatment. Injection of enzyme/corticosteroid or attempt of liposuction with a small cannula are available options. Good luck.

Answer: Diagnosing and treating festoon The area of concern is tricky both in understanding the true ideology, making a correct diagnosis and treatment options. The fullness may represent festoons which is believed to be sagging and bunching up of the orbicularis oculi muscle. The textbook answer for festoons has typically been suspending the Orbicularis muscle. I’ve also heard renowned plastic surgeons talk about this area being prone to having Edema and swelling without known ideology or treatment options. If you’ve had previous facial aesthetic work and it’s very important to get a clear understanding if fillers have been injected in this area previously and or if it has been treated with Botox. In regards to non-surgical facial intervention the actual training report certification probably isn’t as important as simply having inherent skill combined with years of experience and good judgment. When it comes to surgical intervention a plastic surgeon who has a strong history in facial aesthetics versus facial plastic surgeon can be a tossup. In the end nobody really learned how to fine-tune their facial aesthetic surgery skills during residency anyway. You should be prepared to hear different opinions regarding ideology and diagnosis from different providers including different treatment options. It is not an easy area to treat. Answer: Raised Under Fat Pad Treatment What you have is a congenital area with decreased volume above the normal cheek fat. This is easily treated with a filler by an expert who understands the anatomy of the area. Unfortunately, these individuals are rather scarce.
