With proper treatment of gynecomastia, which includes removal of all the tissue present, it is unlikely to recur. No matter what the circumstances, if the tissue that reacts to medication or hormones is no longer present nothing will happen. If you remove the engine in your car and keep filling the tank with gas nothing will happen when you turn the key.The most common artificial cause of gynecomastia recurrence is incomplete removal of the tissue. You can wait for a long time for the "swelling to go down" but you will find excessive fullness from the tissue that has not been removed. This is not uncommon. Treatment would be a more thorough removal of the tissue as a secondary procedure.

I totally understand your frustration, but 5 weeks is really too soon to be passing judgement on your results. You are still swollen and very well could be for the next few months. For now, be patient. If you are still not happy at 6 months post op then speak with your surgeon.
Like everything, there is a scale of "normal". Your nipples are simply on the larger end of that scale. If they really bug you, you could schedule a consultation with a board certified plastic surgeon to discuss nipple reduction surgery, but this will probably also reduce your nipple sensation....
This is typically due to hormonal or weight changes. If there has been no weight gain and the mass has enlarged it should be evaluated by your doctor to insure it isn't something more significant. Don't sleep on it. All the Best, Dr. J
