Have had 3 surgeries, septorhino, sinus, nose correction. Doctor previously ruined the shape of my nose, it was bent with a hanging twisted columella but last surgeon straightened it and lifted the columella. However, it still twists to one side resulting in very visible uneven nostrils and dent in columella. Is it safe to operate again? Would it alter my nose tip? Happy with the tip and the rest of the nose. Just not the columella, dent and uneven nostrils.

This is a complicated issue. There are multiple options to address and notched ala. Another composite graft can be attempted but also an alar rim graft or alar repositioning technique might be appropriate options too depending on the individual factors and exam findings.
