Hi there! A rasping procedure in the OR can help with reducing irregularities. If there's concern for an open roof, it could make the problem worse and widen the nose. Many times, careful palpation in clinic and observation of the bony vault and cartilage in surgery tells a lot. A open roof requires osteotomies and possible cartilage grafting to provide support. Overlay grafts such as fascia can help with camouflage but not support. As for the tip cartilage, at the same time, this could be evaluated and refined if needed using grafts and suturing.
