1.5 year post-op rhinoplasty. Minor revision recommended by primary surgeon. What needs to be done? (photos)

1.5 year post op from septorhinoplasty. A dorsal hump reduced slightly and cartilage placed in depressed area to straighten. Filler was placed a few times. I am recommended a minor procedure in the OR to rasp down the dorsal irregularities. I have concerns that this is an open roof and I would either need a fascia overlay or osteotomy. I don’t want a wide, flat area. I also have concerns regarding cartilage tip that is irregular, however surgeon feels this is fine.
