Liposuction Dimpling/Irregularities- Revision Advice? (Photos)

Hi there, i had liposuction in 2019, and I have some dimpling and "denting" and some odd contours from the lipo on my lower back and flanks. my surgeon has agreed to fix it, by doing more liposuction to "feather" and smooth the areas. 1. Is this a good approach? Is it minor enough, or do I actually need a fat transfer instead? I think he's a great surgeon, but i know he doesn't do a lot of revisions so this plan worries me. 2. What is causing this dark "bruise" like appearance? It's right over an injection site ontop of my buttcrack where the fat was harvested. When i lift up the skin, the darkness dissapears. From the side, theres a little "lump" - how can this be fixed? 3. Will this revision Be tolerable under local anaesthetic? 4. Is it a big deal if I'm 15lbs heavier than I was at surgery? Thank you for your question and photos. In all posted photos you have depressions and excess fat from your previous liposuction. This requires a new operation, "feathering and smoothing" will not work so, #1 will not work. After previous liposuction, there's a lot of internal scarring (depressions) and irregular fat removal. To achieve a good result, all these areas need to be released (I use the VASER ultrasound and PAL technique). The lump at the top of the buttock crack is a sacral fat pad that was not removed. Removal of this is very important to create a more aesthetic feminine buttock. This is not a revision and would not advise to have this done under local anesthesia. Based on the series of nicely posted photos I agree you need revision superficial Lipo-sculpting with micro/nano fat grafting. To answer each of your questions in order: 1. No, feathering Lipo will not address the denting since you need fat grafting with “feathering” Lipo or superficial Lipo- sculpting. It is like sculpting by removing and adding. 2. The hyperpigmentation over coccyx is blood staining or hemosiderin staining. Try lightening solutions but most likely this is a permanent situation. 3. No!, you will neeed light general anesthesia. 4. Not sure best to virtual consult with . Fee for revision. From $6,000-$10,000.
