I had tear trough filler in both my under eyes just over 3 weeks ago and have been left with a puffy under eye I have been back to the nurse who carried the procedure she said it maybe my lymps nodes and i may need to massage area more ive been doing this yet no improvement. She has also recommended i have more filler a bit lower down because of loss of volume may even this it out im just worried that if i do this again another issue may occur and more filler will be required which i didnt really want.

Hi. Thank you for your question. The puffiness you are experiencing can be due to multiple factors related to the filler. It might subside on its own with some time. Or, you can have the filler dissolved with Hyaluronidase and reassess in 2 weeks.

I do not see preinjection photos, but your after photos may show overcorrection, or injection of too much filler. If this is an HA filler, hyaluronidase injections may be considered to dissolve the product, depending on your examination. Too much fullness under the eyes is common after injection in this area but it is not your lymph nodes that are at fault. It sounds like the person who injected you is not experienced and more filler injections may make it worse. I suggest you find a more experienced injector (plastic surgeon or facial plastic surgeon) who can evaluate you and possibly dissolve the filler.

Thankyou for your question.Definitely don't add anymore filler as this will only make the puffiness worse. The puffiness may due to  lymphatic drainage affected by the filler, filler too superficial or tto much filler. All roads lead to the filler so it needs to be dissolved.Its not a lymph node. If your injector will not do this i would go see an injecting doctor.Good luck

Unfortunately this is a common problem and is usually caused by an inexperienced injector. I would suggest seeing a facial plastic surgeon to have this reversed with hyaluronidase. After waiting 2 weeks you can then be re-assessed. I often find that once I have dissolved filler put in to patients like you, the problem is actually loss of cheek volume rather than needing volume in the tear trough in the first place.
If someone has given you lumpy tear troughs, I certainly would not let them inject you again to try and hide it. This seems like a bad idea to me.

This results from too much under-eye filler being placed.  The product hydrates over a period of weeks, then balloons out and swells.  This is easily corrected with an injection of hyaluronidase to dissolve the excess filler.

I would get another opinion from a physician injector.  You may want to consider having the filler dissolved with injection of hyaluronidase.  Under the eyes, I prefer to use a cannula rather than a sharp needle to give the smoothest results and reduce the risk for bruising and other side effects.  Best,Dr. Ort
