Hello, I want to narrow the nasal sidewall of my nose. It feels as though it is an excess of cartilage and not a wide nasal bone. I would also like to refine the tip just slightly. I love my nose from the side profile and do not want to change it whatsoever. How would this procedure be performed? Is a lot of down time to be expected? I have included photos. Thank you in advance!
Answer: Narrowing the nose From the pictures, it looks like it is the nasal bones that are wide rather than the upper lateral cartilages. Osteotomies (cuts in the nasal bones) are performed to narrow this area. If the upper lateral cartilages are wide, that can be narrowed too, but yours looks more bony. As for the tip, it could be refined to be a little less bulbous on frontal view without significantly altering the profile. Be aware, though, that some small change in profile is likely...though that can be minimized. Downtime varies from patient to patient, but I usually recommend taking a week off since there would be an external plastic cast on the nose for six days and patients typically feel like they have a bad head cold (stuffy, runny, etc.) Bruises, if severe, can take up to two weeks to fully disappear so it depends how discreet you plan to be about having surgery.Hope this helps,--DCPPearson Facial Plastic Surgery®

Answer: Wide nose rhinoplasty Whilst you would certainly benefit from osteotomies (see other answers) I actually think a major part of the problem is a narrowness to the middle third of your nose. The osteotomies will partially correct the problem, in order to get a nice smooth and graceful 'dorsal aesthetic line' you will need the middle third widening slightly and this means 'spreader grafts'. A reputable rhinoplasty surgeon will be able to explain this in detail and I would recommend making an appointment with someone local to you. Best of luck.

Answer: My comments Being your profile straight and your dorsum flat from the side view, to whom you request frontal narrowing of the dorsum may be tempted to take the easy ride and make a shortcut: shaving a non existant hump due to lacking skills or training to perfrom a dorsal narrowing WITHOUT dorsal shaving. You would be surprised how few surgeons do know and master the specific technique to accomplish dorsal narrowing without one single mm of dorsum shave, but this method exists and has been described decades ago. So... first advice: do insist, confirm and guarantee the surgeon WILL NOT shave your dorsum but will exclusively narrow the bones.This is done by chisel or manual saw resection of the upper dorsum synostosis (bone fusion) between the nasal bones and the hard septum, and resection of the lower dorsum synchondrosis (cartilagenous fusion) between the upper lateral cartilages and the soft septum.Additionally you have a bulbous, oversized and poorly defined tip, needing reduction, plasties, and mainly tip Sheen's grafting.I must say your nasolabial angle is a bit too high; well it really fits you; you can freely decide to leave the same upturned tip or lower it (columellar graft may suffice for a moderate lowering of the angle).Difficulty 7 out of 10 in primary noses; surgical time 2-3 hours.Please do post or send privately a full set of good quality, well lit and focused standard photos: frontal, both lateral views and both oblique views, also from underneath the nostrils. Feel free to request any additional information from me.
