At the time of breast augmentation, a circumareola approach can be used to shrink the size of the areolas and place the implants. The other alternative is an areola reduction with an inframammary fold incision to place the implants.

Resizing your areola is commonly done with a lift but not really something you should do if you’re just getting implants. There would be a scar all the way around the areolae and sometimes it doesn’t heal well

You need to call your surgeon's office and discuss this now. On the day of surgery, you have signed your permit and nothing extra can legally be added, especially after you receive any preop medication. Plus, additional time may need to be added to the schedule which might be a problem if another surgeon is following your case. Go talk now.

Areola reduction is possible and is a great procedure to be done in conjunction with breast lift surgery, a.k.a. mastopexy or breast reduction surgery.
For individuals not having any type of breast lift there are inherent limitations to how much Areola skin can be reduced.
Taking out too much skin increases the chance of complications, including wide scars, flattening the shape of the breast and the areola stretching after surgery.
This is something you need to talk to your plastic surgeon about. There is no yes or no answer. If you’ve already had the procedure done once then, they may be inherited limitation’s, or it may be possible to reduce your areolas further.
