Back in 2015 I had a BBL done that left a big lump/pouch underneath my belly button. Went back in 2017/18 to have it fixed but so much was sucked out that it left me with lumps, and more fat above my belly button. My question is, should i have it fixed with Sculptra or do a fat transfer again? I paid so much money I feel it should've been done right the first time & I'm so sick of being self conscious and having to Photoshop my stomach. I'm ready to have it fixed by a DIFFERENT doc

Answer: Difficult situation Correcting poorly done Liposuction can be difficult and often not possible to do. This is especially true for those who have been over treated leaving them with two little fat and on top of this and uneven fat contour. As plastic surgeons we can take a smooth even contour and leave it smooth and even as a final result if we know what we’re doing. However, taking an uneven contour and turning it into a smooth contour is many times more difficult and after not possible. Generally treatment options are revision Liposuction, fat transfer for converting this to a skin tightening procedure like a full tummy tuck. None of these are really good options for you for some fairly complicated reasons. You may be better off cutting bait and not spending more money. The single worst thing you could do is have more aggressive Liposuction with power equipment such as VASER. I cannot emphasize enough the importance of patients doing their homework and confirming surgeons have sufficient skill and experience with the procedure before scheduling for body contouring procedures. Liposuction is perhaps one of the easiest procedures to perform but it turns out to be one of the most difficult to perform consistently well. To get an assessment of the amount of improvement possible you definitely need in person consultations. There’s no one in particular technology or treatment that’s going to fix this. It’s always a matter of partial improvements and if that justifies having surgery. Realize that when plastic surgeons take on undesirable outcomes from other providers they inherit all of the problems that come from the previously done procedures. Because of this the general standard should be that each surgeon manages their own patients and takes care of their own complications. We all realize patients lose faith. In my practice I typically offer free consultations for primary cosmetic procedures but we charge for consultations for second opinions of undesirable outcomes from previous surgery including all follow up visits and questions unless someone has surgery with us. Situations like you’re on I highly complex, time-consuming and challenging from both the patient and providers perspective. They can be time consuming and often very little improvement can be offered. The plastic surgeon still needs to be compensated. I wish I could be more optimistic. My recommendation is to get in person consultations by the most experienced and talented plastic surgeons when it comes to Liposuction in your area. Consider writing honest reviews describing your experience so other patients can know what you went Through and the importance of plastic Surgeon selection For Liposuction procedures. Be very careful of those who think that this is easy to fix. I would also be very reluctant in believing that radio frequency treatments are going to correct an uneven fat distribution based on poorly done previous Liposuction.
