I am about 8 months post op from my second bbl. This area will not keep fat. After my first Brazilian butt lift it was fine but turned into this, i got a round two and same thing happened. I do not want to get another bbl because It will be too big. Is there any way to possibly fix this? Will filler be a good option for this area?
Answer: Scar realise and fat grafting One way to improve current deformity is to do meticulous scar realease and fat grafting. Considering that first attempt of correction did not produce improvement, you should consult another board certified plastic surgeon(s) for a second opinion. Good luck.

Answer: Dent 8 months post round 2 BBL Indeed, this can be a very stubborn area post-BBL. One thing I can tell you for sure is, do not waste your money on “fillers”, they will fail and/or be exorbitantly expensive. Depending upon physical exam findings, the only way that this may be remedied is with a lower butt tuck. Please keep in mind that very few board certified plastic surgeons have much experience with lower butt tucks, so you may have to travel outside of your local area to find a specialist. Answer: BBL results To make an assessment on the outcome of a cosmetic surgery procedure we need to see proper before and after pictures. If you’ve had two procedures that we need to see before and after pictures for each procedure. Without knowing what you look like before we can make an assessment on what was done. To me it looks like your surgeon did aggressive liposuction of your upper thigh with the hope that it would make your buttocks look bigger. Doing that you destroyed your S-curve creating when I sometimes called “the lollipop look”.That should have never been removed from your outer thigh. It’s a very bad technique that causes your type of disfigurement. It looks like it was done aggressively so restoring the volume just below your buttocks it’s going to be difficult. The only way to restore volume in the area that was treated with previous Liposuction incorrectly leaving you disfigured is to graft fat in that area building up the volume that was incorrectly removed. Different plastic surgeons have very different ideas of how these procedures should be done or what a female body should look like. I’m sorry that the provider did this to you. At this point if you want to move forward you absolutely have to get documentation for a new surgeon to make an accurate assessment. That includes collecting all before and after pictures and all operative reports. These are part of your medical records and you can get them upon request if you contact each of your providers. Grafting fat in areas previously treated with liposuction is sometimes only partially effective because the previous Liposuction has removed vital host tissue needed in order for grafted fat to survive. what this means is the only way to build up volume in the area is to do conservative sessions of fat grafting slowly building up the volume over multiple procedures. It is difficult and restoring a normal contour may take 3 to 5 procedures. I cannot emphasize enough the importance of properly vetting plastic surgeons before committing to having these procedures. I recommend patients do that by having a minimum of five in person consultations before committing to having surgery. During each consultation ask providers to open up their entire portfolio and show you their entire collection of before and after pictures. Bring pictures of your own body and ask providers to specifically only show you pictures of previous patients who had similar body contours to your own. At this point that may be challenging since your outcome is somewhat unique. The goal is not to find out what the best result of a providers career looks like it is to find out what the average results look like, what somebody with your candidacy results will look like and get confirmation of how many of these procedures they have actually performed. Ask providers to show you examples of excellent outcomes, average outcomes and outcomes that didn’t turn out as well as they had hoped. Ask providers with the revision rate and revision policy is. For reference and experience surgeon should have no difficulty showing you at least 50 sets of before and after pictures of commonly performed procedures. In reality highly experienced surgeons will have hundreds or even thousands of before and after pictures to choose from. The goal from the patient’s perspective is to get an idea of which previous patient to look just like you and most likely to look in other words what do you average results look like in the hands of each provider not the best result of their career. Revision work is many times more difficult than primary liposuction or fat transfer. The importance of choosing the most talented and experience provider cannot be overstated. Some providers may also be reluctant to take on revision work because the possibility of getting quality outcomes is low and the amount of work involved is extensive. Patients who have been disfigured by poorly done previous procedures often are very time intensive consultations and many patients don’t end up scheduling surgery. There are however lots of talented providers who will be willing to help you but without finding a sufficiently skilled provider who can make an accurate assessment with good predictions of what the outcome will be do you always are at risk for poor outcomes. There’s no correct number of consultations patients need to have to find the best provider but the more consultations you have the greater the chance of finding the best provider. I do not recommend patients have virtual consultations and I would highly recommend against traveling long distances for elective surgical procedures unless absolutely necessary.
