Looking for the closest to natural-looking breast method with the least amount of scarring that has the highest chance of preserving the most nipple sensation. I'm open to implants but would prefer that be a last resort option as long as it does not involve vertical scars. And if two procedures are required, would you recommend doing them at the same time or separately, and if it's the latter, how much time in between is recommended?

I would think you could achieve a full breast with fat grafting. We should be successful with another round with adequate volume and proper technique. Seems like DIEP or other flaps would be a salvage option. Thank you.
