The short answer is "yes, there is a risk of a flat butt after a 360 circumferential lower body lift". With the 360 circumferential lower body lift, an abdominoplasty is performed and the abdomen is pulled down flat and towards the center. When the incision is made over the hips, the hips and lateral thighs are pulled up. On the back side, the buttock is pulled up. With the 360 circumferential lower body lift with everything in the front being pulled down and everything in the back being pulled up, there is a risk of becoming box shaped. It is rare, however. Usually with an experienced plastic surgeon, the boxy look after a 360 body lift can be avoided. If you do wind up with a box shape after a 360 body lift there are several procedures that can be done to correct the "box type" look. These includes a vertical torsoplasty where the incision is made from the armpit to the top of the hip or outer thigh. I also call this an "hourglassing procedure" to create an hourglass figure. If the skin is not the problem creating the box like silhouette, you may consider aggressive VASER liposuction over the love handle region. All in all, it is rare to become boxy after a 360 circumferential lower body lift.
