Question

39 yo female with uneven brows due to lack of volume on right forehead. Considered lift, but only one side is bothersome. In photos, the assymetry is more evident. Can filler be used for this? Or maybe fat transfer? I already consistently get tox treatments which helps some.

Yes&mdash;brow asymmetry can often be improved very effectively with nonsurgical treatments, including dermal fillers and targeted neuromodulators. In many patients, brow asymmetry is caused by subtle differences in volume, support, or muscle activity between the two sides of the forehead. Dermal fillers can restore structural support in key areas such as the temples or lateral brow, helping to lift and balance the brow position. In the right patient, this creates a natural, harmonious correction without looking “filled.”
Neurotoxins like Botox or Xeomin can also be used strategically to lift one brow or soften overactive depressor muscles that create downward pull. When combined thoughtfully, filler and neuromodulators can produce a very natural, symmetrical result with minimal downtime.
Fat transfer is another option, but it is more invasive, involves more downtime, and may not offer the same level of precision or adjustability as fillers&mdash;especially when trying to correct subtle, millimeter-level asymmetries.
A proper evaluation is essential, because true brow asymmetry can stem from a combination of structural, muscular, and anatomical factors. I recommend seeing a qualified, experienced injector who focuses on facial harmony and understands the nuances of brow support to determine the best individualized approach.

I have personally been injecting HA fillers for this purpose for many years. So, the simple answer is YES, fillers can be used for this purpose.
Regarding injecting fat, I stopped injecting fat many years ago. For one thing, fat transfer is expensive, requires TWO PROCEDURES, one to remove the fat from the donor site and a second to inject it into the recipient site--each require the use of large bore cannulas (since fat globules are thick), which means more bruising, swelling and post-treatment downtime. What's more, promises of permanent results are seldom achieved since the fat separated from its original donor site blood supply often does not survive. Moreover;, any fat that does survive retains the characteristics of the area from which it is taken. This means that if the fat is taken, for example, from the abdomen when the abdominal fat develops "midriff bulge" with aging, so, too, can the fat injected in the brow. Not a happy scenario. And finally, HA filler can be fairly easily dissolved should the need ever arise. The fat that is transplanted cannot be so easily removed. 
You would be wise to consult with an experienced, board certified cosmetic dermatologist filler injector expert. Hope this helps and best of luck.

In cases like yours, where one side lacks a bit of fullness, a small amount of filler can often help balance the brows beautifully. This can be done very precisely to even out the contour of the forehead or brow area.

Fat transfer is another option, but it’s a bit more invasive and typically reserved for patients seeking a more permanent solution or who are already undergoing another procedure. Since you’re already doing tox treatments, adding a bit of strategically placed filler is usually the next step I’d recommend before considering surgery or a lift.

Of course, the best approach depends on an in-person assessment to evaluate skin tone, muscle movement, and the degree of asymmetry — photos help, but those small nuances make all the difference when planning natural results.

Botox is the most common non surgical tool used to address asymmetry of the brows.  Filler is not used in this area typically due to risk.  You also can consider a brow lift to help even them out. Every single individual on this planet has a symmetry in the face and the body. The eyebrow symmetry is almost  always linked to the asymmetry in the skeletal structure of the face with one eye socket, being a little higher than the other, also being set deeper with respect to the other. That in itself implies that you should not attempt to do anything in an attempt to chase perfect symmetry. You will only create more problems by trying to do so.
