Thinking I might have tuberous breasts, but don’t want areola scars or implants. However I LOVE the shape of my breasts when the nipples are erect. When erect, the domed appearance of the nipple is reduced and the ENTIRE breast seems rounded and fuller and a very different shape. Is there a way to make the nipple/ breast shape like this permanently, through something like inverted nipple/ gynecomastia surgery but for women? If not, can tubular be corrected without areola scars or implants?
Answer: Tuberous breasts? You have very slight doming of your areolae, or at least whilst your nipples aren't erect. Herniation of the areola is a feature of tuberous breasts, but yours are very mild. In my mind you would also need to have constricted lower poles to your breasts to be called tuberous breasts, which you do not have. Yours is just a normal variation of the areola. If you are after surgery with no visible scars then there isn't really a good option that I can think of for you. If your nipples were inverted and you wanted them permanently "out" then nipple inversion surgery leaves almost invisible scars. Your best option would be breast augmentation surgery to fill out the whole breast skin, and leaves only a 4cm scar in each crease. But as you are happy with the size, then I would suggest doing nothing.

Answer: Releasing constricted tissue There does appear to be some constriction of tissue around your areola, leading to a tubular appearance. Constricted tissue can be surgically released to allow for more protrusion of the nipples, giving you a rounder breast shape. No implants are needed, however as with any cosmetic breast surgery, this procedure does have related risks (such as negative impact to breastfeeding ability and changes in nipple sensitivity).

Answer: Mini lift Your breasts are not tuberous, they are simply widely separated. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. Best Wishes, Gary Horndeski, M.D.
