I’m a 22 year old Afro-Caribbean and really want to achieve a smaller defined button nose, is this achievable with my current nose. Pictures 1 and 2 are my current nose and 3 + 4 are what I want.

Answer: Reasonable Goal This is a very reasonable goal, as small changes to your nose will attain the contour/shape you are looking for. That said, I tell all my patients who bring photos of a specific nose they love that no two noses are identical, and it's impossible to get a 100% like-for-like nose with rhinoplasty. For your case you will need your bridge lowered quite a bit, some soft tissue thinning at the tip, lateral and medial osteotomies, and tucking in your infra tip lobule. Likely alar base reductions if you want your nose more narrow on frontal view. This is the general formula to get 90% of the way there, then you just need an expert with a great eye to get you the next 5 - 8%. Hope this is helpful :)

Answer: Rhinoplasty intervention I think your nose looks good, and has no major clear indication for needing to be changed. The Dorsam can be narrowed through in fracturing. I suggest having a few in person consultations with plastic surgeons in your community who have extensive experience with rhinoplasty surgery. I cannot emphasize enough the importance of selecting skilled and experienced providers for this type of work. Look for plastic surgeons, focus exclusively on rhinoplasty surgery, or those who have rhinoplasty as a significant part of their practice and have been doing rhinoplasty surgery for at least one to two decades. I generally recommend patient schedule multiple in person consultations before selecting a provider. During each consultation ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar characteristics to your own. An experience provider should have no difficulty showing you the before and after pictures of at least 50 previous patients. There is an art to doing this procedure well and there’s no substitute for those who have lots and lots of experience with proven track record of consistent quality outcomes. Provider selection cannot be emphasized strongly enough. I recommend people avoid virtual consultations whenever possible, and schedule more than one consultation to help find the better provider.
