While it is more difficult to make a nose longer instead of shorter, it can be done to some degree. Other things may have to be done at the same time, so it's very important to see a rhinoplasty specialist that can do computer imaging at the same time. That way everyone can be on the same page.
You appear to have a pleasing nasal profile appearance as it is. If you were strongly desiring to lower the tip of your nose, you may need to consult with a specialist familiar with septal extension grafting. Sometimes nasal lengthening can be achieved with extended spreader grafts as well. Hi from Istanbul! Yes, extending a short nose can be accomplished with advanced rhinoplasty techniques, allowing for elongation of the nasal tip. (Likewise, deprojection is also possible.) If you wish to extend your nasal tip, so that your nasal tip projects just slightly above the dorsum of your nose, a septal extension graft is an excellent choice as it uses very strong cartilage from your nasal septum to help push your nasal points forward, and because the cartilage is so strong, the results are very stable and durable. Yes you can extend your short nose. You would need to have a rhinoplasty that involves placing a septal extension graft. This may or may not be accomplished using just the cartilage in your nose, just depends on how much you have. Bear in mind that when you are lengthening the nose, you are putting the skin on much more tension so it's a little bit riskier in terms of healing but I think you could get a great result.
Your concern is visible in the photo, and you have done a good job demonstrating the ideal correction. Elongation of the short nose is possible, and is limited by two factors. 1. The ability of the nose skin to expand (elongate), and 2. The availability of cartilage graft to extend the length of the nose internally. Another way to think about this is that the nose skin drapes over, and takes the shape of whatever scaffolding is built under it. In most patients, the skin expansion is not an issue, except in certain cases that do not apply here. Cartilage graft for the extension of the internal structures can be harvested from the septum, the ear, or even from the ribs. Recently, sterile donated cartilage grafts have also become very effective, as the processing techniques have improved. Therefore, it is very likely, that you can achieve the result you have shown.Dr. B.

Most western plastic surgeons are generally taught to avoid nasal implants because of high complication rates.Augmenting the nose is considered more difficult and complex than nasal reduction surgery.I suggest consulting with plastic surgeons in your community who seem to have extensive rhinoplasty experience.Ideally look for plastic surgeons who have one to two decades of experience doing rhinoplasty surgery.Ideally look for plastic surgeons who focus exclusively on rhinoplasty surgery or those whose practice has a high percentage of rhinoplasty surgery.I generally recommend having in person, consultations whenever possible, avoiding virtual consultations.It’s generally better to not travel long distances for elective surgical procedures, but sometimes travel maybe necessary in order to find providers who have sufficient skill and experience.Being board-certified in plastic surgery with years of experience, and an overall good reputation does not mean somebody has mastered every procedure.
