If you're looking for more fullness an implant is the most reliable way to achieve this. Fat grafting can help but tends to be more subtle. I'd recommend getting a few consultations by board certified plastic surgeons and see what they recommend. Look at their lift vs implant/lift results and see what matches what you're looking for.

Dear Fierce169488,determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. Thank you for your photographs. From your photographs, it appears that you have a ptosis of your breast with a portion of your breast hanging below the fold underneath your breast. A breast lift would help to tighten the skin and shift the breast tissue superiorly. By removing some of the skin of your breast, but keeping all of the other volume, that same amount of volume pushed up inside a smaller envelope of skin will help to plump out that skin envelope and make the breast appear fuller and more youthful as well as sitting slightly higher on your chest.
