I am 37 & seem to have prominent nasolabial folds and marionette lines. The marionette lines bother me the most. I have tried Juvederm in the past a few times, but that didn’t seem to help as expected. Felt I wasted money. So my Dr. suggested Sculptra. I have done one session of Sculptra (2 Vials) 1 month ago & going for a 2nd session soon. (Another 2 Vials). Is this going to help or am I wasting money? Do I need a lift at my age already? How Can I get rid of these! Ugh!

Thank you for sharing your question and photographs.  Sculptra will be able to help improve your smile lines and marionette lines if placed appropriately.  Your volume needs to be over the apples of your cheeks as formation of the creases is due to cheek descent.  Lifting of these tissues into a higher position - like in your smiling photograph - is the goal.  Hope this helps.

I think that Sculptra is a good choice for you.  Sculptra addresses the underlying fat and bone loss in the upper face which is what causes prominent nasal labial folds and marionette lines.   The downside to Sculptra, is that it is not as instant as some of the HA fillers, but in the long run is longer lasting and addresses more directly the undelrying cause.  After completing your initial Sculptra injections, you may still need some HA fillers in the area, but it will be significantly less.

You may want to try a filler that has more "lift" to it. Radiesse is more powerful in this regard than Juvederm or Sculptra. See link below for examples.

Your photos show that the area that would be best treated is the mid face. This can be treated with either Sculptra or Voluma.  It would take a few treatments of Sculptra to get the desired results.  Voluma is much more immediate and perhaps you would like that better since you could have the results that you want after just one session.  I would also combine Thermage with these treatments.  Please consult an expert in facial injections for the best cosmetic results.
