Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.Frankly, i have had the best success with scar excision revision and buttock implants to “pop” the scar out. The result will never be perfect but much higher long term success than any fillers or fat transfer. Please note that an extremely few number of board certified plastic surgeons specialize in buttock or hip implants, so you may need to travel far and away from home to find this particular surgeon. Although it is difficult to say with complete certainty since I have not examined you, improving depressions on the body made by scars is usually a multi-step process and may require staged procedures for best results. Usually the first stage is to surgically release the scar from underneath (or break up the bands of scar tissue that are causing the depression). This is also usually followed by multiple rounds of fat transfer to the area in order to fill in the depressed areas. How many times this needs to be done to get optimal results is different for each patient and depends on your anatomy and the degree of scarring. Make sure to consult with a board certified plastic surgeon, good luck!

The deformity on your buttocks is difficult to treat. It will require exercising the scar tissue. It cannot be corrected by simply adding volume with procedures like fat grafting or fillers. it requires removing the scar tissue and that’s pulling on the skin first and foremost. These are complex procedures that may give some improvement but are unlikely to restore your bottom to normal.To get a better understanding I suggest having a few in person consultations with board-certified plastic surgeons.
