Is the photo before any surgery or after a rhinoplasty?Turning down the nasal tip(derotating the tip) does require a full understanding of nasal anatomy as well as cartilage graft material--septum or ear in order to make the nose architecturally stable over the long term.With experience it is a highly predicable and aesthetic surgery.Sometimes I will use temporary filler to show the patient how their nose will look after surgery.Hope this helps
Once your nostrils have been reduced it is impossible to reconstruct them without visible scarring. At this point your nostrils do not look bad overall. The risk of a revision includes nostrils looking worst than before. It is safer to leave it alone. Regards Dr. J
