There are various approaches to alar narrowing. In cases where not much narrowing is needed, I sometimes use a "cinching" suture without any excisions at all. Also, there are just wedge excisions of the alar walls which reduce the flare but do not narrow the nostril base. And of course, there are minimal excisions, as well!
An ajar excision if one millimeter is possible but it is unlikely that it would create much of a difference. Usually around 3 mm is more like it
Alarplasty is certainly the surgery of millimeters. 1 mm is so small, I find it hard to believe that a 1 mm nostril reduction would lead to a visibly improved result. I hope this is helpful and thank you for asking this question. Sincerely, Dr Joseph
When I measure alarplasty incisions every mm counts. There are times I measure to 0.5mm changes as well. As surgeons we can do it well because we use magnification glasses called Loupes. RegardsDr. J
? is it possible to have a 1 mm alar plasty incision? Unfortunately this is not possible. An alar plasty involves removing a section of nostril that is usually 2 to 5 mm wide. The incision to do this is at least 1 cm long which is about 10 mm. 1 mm is about the size of a pinhead. There’s no way to remove any visibly significant amount of tissue with a 1 mm incision.
It's difficult to project what you would consider as a "perfect" endpoint. You do have a tad amount of excess nostril show, but I don't think that narrowing the nostrils via an alarplasty is the answer. I would suggest lowering the nostril rims slightly, and maybe even adding rims grafts...
Hello! Great questions and thanks for your photos. You can bring the columella lower and refine its scar in addition to nostril symmetry improvement. Nostrils are more difficult to get just right but improvements can be made. There are a number of techniques to improve the nostrils such as base...
Yes, I think you are a very good candidate for an alarplasty to reduce your bilateral flaring. In your case, you do not need any of the nasal floor excised, but just the alar walls!
