Follow-up question: 1 year post-op tear trough filler, why are my eyes still swollen? (photos)

Hello, thank you for your question. The lower eye lid area has complex anatomy and swell easily if traumatized. Filler placement has to be concise and meticulous. If it is placed too superficially or in an area where lymphatic drainage is less effective, you may either have an overfilled look, a bluish tinge (Tyndall effect), or persistent swelling.While an in person consultation with a qualified and experienced injector is the best way to assess what is going on, on looking at your photos you do appear overfilled in the tear trough area. This could be improved with hyaluronidase to melt the Revolax. If you think this is due to swelling, you could try taping the area while you sleep and seeing if it looks better in the morning. I hope this helps. - Dr. Leong

The amount of revolax used and the quantity seems to be just fine. The only other issue left is where the product was placed. Too superficial???? You can get it dissolved with hyaluronidase since it is an HA based product.

From your pictures and the filler used, it appears that your injector did what many are still doing and injected the wrong filler above the orbital ligament. This caused interference in the eyelid lymphatic drainage and, thus swelling of the eyelid above the ligament. If recognized early, the filler can be dissolved and the swelling will dissipate. However, at this time, it is improbable that hyaluronidase will work. You simply need to wait until the filler dissolves on its own. From your pre picture, I would suggest you do nothing else in the near future.
