Are there any unwanted side effects from an internal mesh? Considering that textured implants have a small chance of causing cancer, I was wondering if this is the case for the mesh too. Also, does it really work? If yes, why is it not included in the standard procedure of a breast augmentation?

Any of the implants you are considering should give you a nice result. I encourage you to return to your surgeon to further discuss your goals for surgery. Many have sample implants you can "try on" to give you a feel for how you may appear after surgery.
From the submitted photo it does appear that your implant is bottoming out. This can be seen by the nipple pointing more upward, as you mentioned, as well as the inframammary incision moving forward. I encourage you to return to your surgeon for an evaluation to discuss your options.
Please do not go bigger or you will just sag even more from the excess weight. Your large heavy implant is sagging your breast down from the weight. I would suggest going down in size so your lower pocket can be tightened and help lift you breast back. Fat transfers to the upper breast can also...
