I have opted to have a BA; however, I am having trouble determining the size I want. I was told my chest wall measures 12.5cm and have been suggested either 275cc (classic) or 325cc (M+) to achieve more volume to maintain a natural appearance. I do understand the differences between the types, but is it possible I could still achieve a natural appearance by increasing the implants to 320cc (13cm)? Am I risking having too wide of a breast, too large of an implant, too much side boob?

From the submitted photo it does appear that your implant is bottoming out. This can be seen by the nipple pointing more upward, as you mentioned, as well as the inframammary incision moving forward. I encourage you to return to your surgeon for an evaluation to discuss your options.
Please do not go bigger or you will just sag even more from the excess weight. Your large heavy implant is sagging your breast down from the weight. I would suggest going down in size so your lower pocket can be tightened and help lift you breast back. Fat transfers to the upper breast can also...
I would suggest considering going down to around 350 cc's which will be a better fit as you age and less weight on your neck and shoulders. D's are fairly heavy for someone your height and weight.
