I’m 5’5, and fluctuate between 270 and 300 pounds. I’ve lost about 120 lbs in the past two years after having the gastric sleeve. My main insecurity is the shape of my butt. It’s always been rather flat, but I have a hump, then it goes flat. It’s the one thing I have never been able to get over. I love my thighs, boobs, and honestly don’t mind my stomach. What kind of procedure can fix my flat butt and give me the nice slope I want? Also flatten my stomach.

Answer: Body contouring Dear Easygoing, Post massive weight loss body contouring surgery is becoming very popular and very common with the advent of bariatric surgery procedures and FDA approval of weight loss medications. It is usually an extended, staged process where a few areas of the body are addressed during each stage of contouring surgery. The shape of your buttocks is a commonly seen one following massive weight loss. Autologous fat transfer gluteal augmentation (BBL) will enhance the appearance of your buttocks immensely. Expect that your might need more that one surgery to reach your final shaping destination. Seek consultation with an American Board of Plastic Surgery board certified plastic surgeon that performs BBLs regularly for an examination and discussion of your goals and options. Good luck!

Answer: Treatment options At your current weight, you are not in good candidate for elective cosmetic surgery. Spite previous success with weight loss. You still have a ways to go before your healthy BMI. Most people who had significant weight loss will have skin laxity as their primary issue. Dealing with the skin laxity is probably the number one issue. Skin laxity lowers your potential outcome for operations like liposuction and fat transfer. Individual providers may have different opinions. I suggest focusing on your overall health and continued weight loss as your first and primary goal. Once your BMI is a healthy weight that you can schedule in person consultations with plastic surgeons in your community to get a better understanding of treatment options. Best, Mats Hagstrom, MD
