3 months is still too early for a revision, although some patients opt to have the surgery even at just three months. Many factors may play a role in the final results of breast augmentation. Chest wall measurements, tissue elasticity, the desire results and the technique and skill of your surgeon. 

I'm sorry to hear you're not happy following your surgery. Revision certainly is possible, although it doesn't look to me that the problem is one of breast width - for that it would be worth knowing your breast base dimensions compared with that of your implant. However it appears to me that the implant position is a little high which is making the nipples appear too low relative to the breast. That kind of appearance sometimes happens early after surgery but usually settles by three months. If it doesn't then the implant position can be lowered and the pocket adjusted to accommodate this.
