It's very embarrassing for me. I know I'm overweight but its beyond my double chin thats wrong here.. Would chin implants fix it? It seems to look the most pronounced when I smile

Thank you for your question and sharing your photographs. The reason that the problem shows more when smiling is the pattern of shape change when your chin muscles contract.

In some people it leads to the chin appear more elongated, in some people it looks like the chin is lifting up more prominent forward and in some people it looks like it is just got more wrinkles.

Chin implants do not always improve the situation but botulinum toxin (Botox or equivalent) is an excellent starting point. It will block the movement of the muscle when smiling and preserves the shape of the chin closer to when you are relaxed.

I suggest that you discuss this with a provider that offers botulinum toxin treatments before considering surgical intervention.

In order to give you advice, I would need profile photos and photos where you are not smiling. Sometimes a custom chin implant is recommended and other times botox can be recommended.

You might consider filler to the "pre-jowl" sulcus. This will soften the appearance of the chin and actually make your face appear slimmer. Hope this helps!
