Hi Bobsdog, Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift. Breast lift surgery aims to re-volumize the breast with either and natural implant or fat grafting followed by nipple repositioning using the lift. I prefer to use a vertical incision (lollipop pattern) breast reduction technique instead of the anchor scar pattern for more significant lifts. This reduces more noticeable scarring than usual. Mastopexy that requires less than 2cm of lift only needs a donut incision. Every incision is also closed in five precise layers without using reactive sutures that can cause inflammation and eventual scarring. If you are considering a breast lift, you should consult with a board certified plastic surgeon to discuss your options and make sure your surgeon understands your goals. Thank you for your question. You would definitely benefit from a breast lift, which reduces the skin envelope while reshaping the breast tissue into a more youthful and perky shape. There are several techniques available for breast lift, and it can be discussed during consultation which one is best for you. All techniques are designed to elevate and improve the position of the nipples while reshaping breast volume to give a lifted and perky technique. That said, you definitely have to choose your surgeon correctly and make sure that he/she has the training and experience to meet your goals with absolute safety. A key quality indicator is Board Certification by the American Board of Plastic Surgery. Don’t settle for less than a Board Certified Plastic Surgeon with specialized expertise in this procedure.

Hello and thank you for your question. Thebest advice you can receive is from an in-person consultation. With thatbeing said, based on your photographs, the best shape and contour could beachieved with a vertical or wise pattern breast lift. I rarelyrecommend a Bennelli or circumareaolar (donut) lift. Over time, theareola tends to enlarge with a thickened scar. The other even worseproblem with a circumareaolar lift is that it flattens out the shapeof the anterior breast over time. A vertical or wise pattern lift can result in a very nice shape if good technique isused. Make sure you specifically look at before andafter pictures of real patients who have had this surgery performed by yoursurgeon and evaluate their results. The most important aspect is tofind a surgeon you are comfortable with. You appear to be a good candidate for a Bilateral Mastopexy. It is the only procedure. Be sure to seek an experienced Board Certified PS for in person exam
Breast surgery for those in adult entertainment can be more challenging as all scars can be visible. Breasts have ptosis from more skin than the breast volume will fill. Increasing the volume through augmentation, reducing the skin through a lift or both improves ptosis. With some patients an increase in volume is all that is needed. If a small amount of skin reduction needs to be added then a periareolar reduction can work. Where the ptosis is greater a scar under the breast, from this scar to the areola and around the areola is needed.With the degree of ptosis shown in the photos more scaring or some ptosis will likely be the choice.

Dear Bobsdog,Many thanks for the question and photo. Looking at your pictures you would certainly be a candidate for a breast uplift (Mastopexy). Given the size of your breasts and level of ptosis (droop) I think you would be best served with an inverted T scar to give you a worthwhile result.I note your concern about scarring, however the scars with time should fade to become virtually imperceptible - this is obviously dependent on the biology of your skins healing properties.I wold recommend you consult with a Board certified Plastic surgeon who can examine you and discuss options with you.I wish you well on your journey.BWMarcus
Hi, and thank you for the question Based on the information and photos you send us, you are a great candidate for a full breast lift, since i know that your main question its about the scar, the scar for a lift like yours should be a inverted T scar or a L scar. and i dont see the need for implants in a case like yours, if you are wondering about that. As you can understand studies and test are needed to see if you are clear for surgery but in general, if you seek the attention of a certified plastic surgeon, you should have a great result and a normal life.
