In the photos, there appears to be fullness along the side/flank area near the tummy tuck scar, though an exam is needed to tell how much is pinchable fat versus skin laxity or scar tethering. Because you have already had an abdominoplasty, the surgeon should evaluate scar position, tissue thickness, skin quality, and blood supply before deciding on additional liposuction. If the tissue is mostly soft, pinchable fat and the skin has reasonable elasticity, conservative flank/lateral waist liposuction may improve the contour. If there is significant loose skin, a high or tight scar, or tethering, liposuction alone could leave irregularity or worsening looseness, and a revision skin procedure may be more appropriate.
