I recently had a BBL done and I don’t think I gained enough weight for me. I like my results but it just wasn’t what I was going for. I am wondering if I can regain weight for about a year and go round two?

One of the main considerations when evaluating a patients’ candidacy for BBL surgery is whether they have enough fat stored in their body, so the plastic surgeon can harvest enough fat cells to use during the fat transfer phase. For patients with insufficient fat deposits, the plastic surgeon may advice a weight gain program before BBL to qualify for the procedure. Usually, a highly skilled and experienced plastic surgeon will achieve around 65% to 85% survival rate of the fat cells. There may be a percentage of fat that does not take, which is why touch-up procedures are sometimes necessary. Moreover, patients can actively contribute on the success of the procedure and longevity of the results by following their surgeon’s recovery recommendations. I typically suggest my patients avoiding cigarettes and heavy drinking because these habits reduce circulation throughout the body, leading to the degradation of new cells. Another fundamental measure is wearing specialized compression garments around the clock (except for showering) for six months, to reduce swelling and help their skin contract smoothly to their new body’s contours. In my experience, patients who successfully avoid sitting or putting pressure on their buttocks or stretching for the first 8 weeks after BBL surgery are much more likely to have long-lasting results than patients who fail to follow this advice. The volume and projection can likely be improved, but it will depend upon who did the first BBL and how much fat is left to transfer. I have performed secondary BBLs on patients who wanted more fat transferred. You may have to consider upper arms, medial thighs, and posterior thighs as well. No studies have been done on this, but on the secondary BBLs that I have performed the percent the fat that lives is around 30 to 50%, similar to the primary operation. There is no reason that fat should not live at about the same rate. Wait six months to a year before scheduling the BBL revision.Gaining weight is often counterproductive. If you gain weight that is fat, some of the fat will likely be associated with the buttocks. In almost every case, I do not recommend weight gain prior to a BBL. I think it is better to maintain your stable weight. Recommend that you seek advice from the plastic surgeon who performed your first BBL, as he/she will know your distribution of fat for a second round.

For example, if you weight 110lbs and gain 20lbs for surgery (weight of 130lbs), you must stay at 130lbs after surgery to keep the results of the surgery. If you then gain more weight for a 2nd BBL, you must also keep that excess weight on long term. Will you be happy at that new weight forever?
