I had the “old fashioned” lipo method performed 8 years ago and did all lymphatic massages but still ended up with ab fibrosis . I was turned away by a reputable surgeon who said i would need a reconstructive surgeon. Can fibrosis can be corrected with vaser lipo, or J plasma, or would I need a tummy tuck? when I sit the tummy skin bulges and looks pouchy and my pubis area is fattier now. Would like to even this out.Thoughts?

Answer: Uneven after liposuction Dear hhtexasgirl, it is hard to tell for sure without an examination. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations. Answer: Correcting previous liposuction outcomes Correcting previously done Liposuction is difficult and sometimes not possible. Your provider didn’t have the skill or experience to do liposuction correctly. Fat was removed in an uneven fashion leaving you with an uneven fat distribution. You might get some improvements through revision Liposuction. Primarily this involves removing more fat in under treated and untreated areas like over the rib cage under your breasts etc. There is no way to correct areas that were over treated previously. Fat transfer may be an option but it typically requires multiple treatments and the results are never as good as pre-liposuction. I would stay away from energy devices like VASER and J plasma. My first hand experience with J plasma was highly unimpressive and I have since sold that equipment. Much of the liposuction disfigurement is above your bellybutton and for this reason converting this to a full tummy tuck may not be an ideal solution. Converting Liposuction to a full tummy tuck is often an option for those who’ve been disfigured on their lower abdomen only. There is no particular correct device. There is also no particular device that causes these type of results. It is strictly related to how the procedure was performed or in other words who did the surgery. Liposuction always has been and still is performed with a small stainless steel tube connected to a source of vacuum. There is a lot of hype and spin with different brand name equipment but in the end it is the surgeon not the equipment that determines the outcome. Be reluctant of providers who believe this is easy to fix. I would also be very reluctant to those who recommend breaking up scar tissue using energy devices. More surgery especially energy devices leads to more scar tissue. Do you need accurate assessment and this requires in person consultations. I continue to encourage people to confirm surgeons competence and experience before having any body contouring experience. This is even more true for revision work which is many times more difficult than primary Liposuction. I suggest finding the plastic surgeons in your community who have the most extensive experience and best reputations for liposuction work and following up with in person consultations. There is no reconstruction for badly done liposuction. some improvements can be made but evening out the results but that is the limitation. Best, Mats Hagstrom MD
