Inverted v deformity or open roof? Will it get better?

I am now 7 months post open rhinoplasty and the bridge of my nose seems to be of some concern for me. It appears wider than before my rhino and there are many irregularities that can be seen in certain lights. Can it get better or will a revision be necessary? Anything helps thanks!

After 7 months, the majority of the swelling should be gone. If there are persistent irregularities after this time they could be corrected with injectables along the dorsum which could be a band-aid on the underlying issue of an open roof. Correction of an open roof may entail revision osteotomies, likely medial and lateral if the nasal height is correct or a dorsal on-lay graft, or both.

It is hard to tell from pictures alone.  I would recommend an exam to know if any of these conditions exist.

At 7 months after rhinoplasty the majority of your results should be seen. Your photos reveal some irregularities along the dorsum but it is not clear if you have "an open roof" deformity which an incomplete osteotomy where a prominent "hump" is removed. I would consult with your surgeon and decide what can be done to correct any irregularities you have. Options include filler for mild irregularities to revision surgery.

Open roof and inverted v is more or less the same thing. Surgery is needed to fix it.  It will not go away on its own.

I would recommend talking with your rhinoplasty surgeon. Usually, and inverted V does not improve, and may require revision to correct. The revision can be relatively minor or more extensive given the severity of the deformity. Best of luck to you!

It is difficult to answer your question without the opportunity to examine you; in fact both problems may be present. Regardless, it appears that the best approach would be repeat osteotomies combined with the placement of spreader grafts.
