Would dermal fillers be appropriate or an implant? (Photo)

Question

I have a very asymmetrical face and I'm wondering how to improve it. Would I be a candidate for facial fillers or an implant? I'm not sure what would be needed. I'm thinking cheek and jaw but open to suggestions.

Thank you for your question—facial asymmetry is very common, and it’s something I see often in practice. No face is perfectly symmetrical, but when the imbalance becomes noticeable to you, it’s reasonable to explore options to improve harmony and balance.

For many patients, dermal fillers are the first and most appropriate step. Fillers allow us to subtly add volume where it’s lacking and create better symmetry in areas such as the cheeks, jawline, or chin. One of the advantages of fillers is that they are adjustable and reversible, which makes them a safer way to “test” changes and fine-tune proportions over time. They also allow for a more customized, conservative approach, especially when asymmetry is mild to moderate.

Facial implants, on the other hand, are typically reserved for more significant or structural asymmetry, such as a very underdeveloped chin or jaw. Implants are permanent and can provide strong projection, but they involve surgery, longer recovery, and less flexibility once placed. Because of this, they are not usually the first option unless there is a clear skeletal imbalance.

Based on what you’re describing—concerns about the cheeks and jaw—many patients achieve excellent, natural-looking improvement with strategic filler placement, sometimes combined with neuromodulators (like Botox) if muscle imbalance is contributing to the asymmetry.

The key is addressing balance rather than perfection. A thoughtful, stepwise approach often leads to the most natural and satisfying results.

There are techniques that may be employed to lift the corner of your mouth for improved symmetry, however, we do not have any experience with them. Additionally, your right cheek may be a bit flatter than your left. Depending on your examination, injectable fillers may be considered for your right cheek to improve upon facial symmetry. From my professional experience, I find that while implants offer a more permanent solution, they also come with permanent complications such as scarring around the implant area. Additionally, as one ages, the implant may become more prominent, leading to a less natural appearance. This is especially noticeable in individuals with a normal or low body mass index who may appear more slender. Consequently, some patients end up seeking dermal fillers to camouflage these prominent implants, which can be quite challenging. Often, it might be more prudent to consider removing the implant rather than continuously attempting to disguise it with fillers. Both implants and fillers have their pros and cons, and the best choice depends on individual cases and long-term aesthetic goals. We are all asymmetrical to some degree and you certainly fall within normal limits.  Fillers are a great way to improve asymmetry with immediate results.  Other benefits include being able to go slowly and build to the look you are wanting, as well as the option of dissolving them if desired (for HA fillers). The amount of facial symmetry you have exists in every single individual on this planet. It was simply your choice to start examining yourself to a detail. If you pay just as much attention to the very next individual, you will see the same. Facial symmetry to a certain degree is what makes a person, beautiful on the outside. Total symmetry makes an individual look very strange just like the statues in the wax museum. It is not just a soft tissue and bony protrusion that are uneven. You will see that your left eye is said deeper than the right by approximately half a centimeter and you cannot , correct that. This symmetry plays a very important role. Should you still decide to treat this, I would suggest using fat grafting This is the most natural way and is much better than fillers. Bad grafting provide permanent and natural results.
