Some bruising and swelling is to be expected after the procedures you underwent. It's too early to worry about anything that might require a revision. Management of the eyelids in the interim should be managed by your own surgeon.

You do have one area of the left lower lid that is more swollen than the rest.  This makes it appear more red and inflamed.  You also have some residual bruising.  I would use warm compresses to the lid and do some massage over the raised area on the left.  This should help flatten it out. If it does not go down, your doctor may want to inject it with a little steroid.  You look pretty good though and I think will have a good result down the road.  Fat transfers can be a bit stubborn when healing!      

The patient had eyelid surgery 5 1/2 weeks before and now has swelling and hyperpigmentation in the lower eyelid the area. Fat grafting was done but no mention was made of a postop hematoma. Swelling in the lower eyelids commonly takes longer to recede as contrasted to other parts of the face. Fat grafting can be challenging and localized swelling as well as bruising is not uncommon. Time as well as massage and perhaps topical treatment such as arnica montana cream can be helpful. Fat grafting introducers the possibility of lumpiness because fat globules are difficult to smooth as contrasted to hyaluronic acid filler material such as Juvederm or Restylane. Patients should return to their plastic surgeon for follow-up and perhaps more treatment if they have concerns. This patient is far enough following the surgery that further treatment, perhaps very minor can be done to improve the situation.
