What is the procedure if I want new implants? Do you remove them and insert new, or create a pocket to give a lift?

Hello so I have had implants since I was 19 years old. I just turned 50. So I have had my implants roughly 30 years. I can’t complain about them really as the only thing is now they are starting to sag somewhat. If I wanted new implants what is the procedure. Do u remove old ones and insert new or do u have to create pocket in order to give the lift. Not sure if replacing new implants gives the lift or not. Thank you.
