"Alien" like a bulge on stomach that doesn't want to go away post TT. (10-10.5 months post-op). What is going on? (Photos)

Had a massive dogear post TT, muscle repair & panniculectomy. My surgeon doesn't do lipo at the same time. A weird "alien" developed basically since the beginning. As my muscles come back it's getting worse. I body build, track macro's etc, I haven't gained weight, I can't lose either. I've had 2 rds of lipo in office now. This time on my flanks after the first one didn't get rid of the muffin top issue. Finally, get rid of that and now this alien is appearing worse than before. What is this?
