I recently reviewed my medical records and observed the the practitioner who did my injectibles indicated the same Lot #/Expiration number for fillers that were injected two years apart. I am asking this question as I want to confirm that I was not injected with fillers that had not expired since I had complications with these fillers. Thank you..

Every office has their own consent forms to give the patients information about the procedures they perform. There is no standard consent. I use one form for all brands of Hyaluronic Acid fillers, another for both Botox and Dysport, another for all types of Laser and Lights, etc....
