People's noses tend to droop with smiling due to upper lip muscles that pull down the nasal tip with smiling. We have not seen good results trying to lift a nasal tip with Botox or fillers. The tip of the nose may droop with smiling if you have an overactive depressor septi nasi muscle. Signs of an overactive muscle also include shortening of the distance from the nose to the lip and a horizontal crease underneath the nose. You can try Botox to block this muscle and see if it decreases the amount that your nasal tip droops with smiling. If it does work, keep in mind that Botox is temporary. Cutting or repositioning the muscle with surgery would give a longer lasting result.
